December 5 Pizza Order

Lunch order form for Papa John's Pizza

Each order will be attached to a person (much like the affiliate meals orders for tournaments). Each person may order more than one slice if they choose; however, this form should be filled out for EACH PERSON who is ordering. This makes the process much easier for the person who has volunteered to order and distribute the food. 

We will order pizza from Papa John's. Here are the details:

  • Each slice of pizza will cost $2
  • You will have the choice of cheese, pepperoni, or sausage pizza 
  • A water bottle will be included with the meal

All orders are due by 10 PM, Friday, October 31.
